Passport Information
Minor Passport Information
Cards Minor: (UP TO AGE 15, BOTH PARENTS MUST BE PRESENT)
- Certified copy of Birth Certificate and a photo copy
- Both are sent to Department of State.
- Certified copy of Birth Certificate will be returned to you by the Department Of State in a separate mailing from Agency.
- ID of minor (if they have one) and a copy of front & back of ID, copy of ID is sent with application
- ID/License of both parents and a copy of front & back of ID copy of IDs are sent with application
- We will take photos for a charge of $15.
- Passport card is good for 5 years.
- Cards are good for air/land/sea travel across the border.

EXPEDITE SERVICE IS AVAILABLE FOR ALL PASSPORT OPTIONS FOR A $60 DOLLAR FEE
EXPEDITE MAILING SERVICE TO AND FROM AGENCY IS AVAILABLE FOR PASSPORT BOOKS ONLY FOR AN ADDITIONAL FEE OF $25.98 PAYABLE TO CLERK AND $16.48 PAYABLE TO THE DEPARTMENT OF STATE.
